Complete New Patient Forms: Before your visit, fill out the required new patient forms. These will include health history, medications, and any current medical concerns. You can complete these forms online or at the clinic before your appointment.

Bring Necessary Documents: Bring your identification, insurance card (if you have one), proof of income for sliding fee scale eligibility, and any medical records from previous providers.

Payment Options: If you don’t have insurance, our Outreach Team can help you apply for Medicaid, CHIP, or other assistance programs to ensure you get the care you need. We also offer a sliding fee scale for those who qualify.
