Outreach Services

The La Esperanza Clinic Outreach team dedicates their daytime efforts to screening patients for a range of programs addressing crucial needs such as insurance, food, housing, utilities, continuing education, and job training. They specialize in connecting patients to programs, services, and organizations that may be unfamiliar, offering support in both English and Spanish. Assistance with applications is readily available, ensuring a smooth process.

Beyond Basic Assistance

The Outreach Team goes beyond basic assistance, providing hope and peace of mind to patients facing various challenges in their lives.

  • The Outreach Team offers comprehensive application assistance, guiding individuals through the process for Marketplace and Your Texas Benefits, covering Medicaid, CHIP, Medicare Savings, SNAP, and Healthy Texas Women’s programs.

  • Through the Patients Assistance Access Program (PAAP), the Outreach Team provides vital support, including assistance with the food pantry and access to care coordination/case managers for the dispersal of durable medical equipment and bus vouchers.

  • The Outreach Team actively connects patients with local agencies and offers application assistance for housing and utility programs, ensuring they receive essential support. The team collaborates with community partners to extend these resources.

  • Engaging with the community and patients, the Outreach Team actively participates in health fairs, speaking/education engagements, and collaborates as members of various coalitions, committees, and task groups to enhance healthcare awareness and access.

Carina Sturgeon is an accomplished professional currently completing a Master of Public Health, holding a Master of Arts and a Bachelor of Arts. She has been honored with the Texas Association of Community Health Centers Outreach and Enrollment Champion Award in 2023 and the Concho Valley Adult Protective Services Board APS Honors Award in 2019. Carina's journey with La Esperanza Clinic began over 10 years ago, when she attended her first TACHC Conference and was inspired to pursue a Master of Public Health. In her downtime, she enjoys traveling, attending concerts, playing with her puppies, gardening, and spending time with friends and family. Carina considers herself fortunate to work at La Esperanza Clinic, where she can fulfill her passion for helping people every day. She relishes the opportunity to use her expertise to educate patients on policies and resources available at local, state, and federal levels.

Esmeralda Perez is a dedicated Community Health Worker who graduated from Howard College in May 2023. Originally from Del Rio, Texas, Esmeralda's upbringing in a family of seven instilled in her the importance of advocating for those in need. She moved to San Angelo five years ago and is passionate about working with all patients, especially expecting mothers, teen families, and children. She finds great fulfillment in assisting the Spanish-speaking population who often feel unheard due to language barriers. On sunny days, she loves going to the pool with her kids and cherishes quiet moments watching movies with ice cream. Her patient care philosophy centers on the belief that true patient-centered care means addressing the whole person, not just their immediate health concerns. Esmeralda helps patients navigate the complex web of Medicaid, SNAP, and other social services, ensuring they feel supported, empowered, and never alone in their journey to wellbeing. Esmeralda chose to work at La Esperanza Clinic because of its emphasis on preventative care and health education. She believes that empowering people with knowledge and resources to manage their health proactively is key to building a healthier community. The clinic's dedication to providing accessible, affordable services to underserved populations aligns perfectly with her values.

Application Assistance

The Outreach Team offers comprehensive application assistance, guiding individuals through the process for Marketplace and Your Texas Benefits, covering Medicaid, CHIP, Medicare Savings, SNAP, Healthy Texas Women's as well as other programs.

Marketplace

Shorthand for the “Health Insurance Marketplace®,” a shopping and enrollment service for medical insurance.

Medicaid

Medicaid is an insurance program that provides free or low-cost health coverage to some low-income people, families and children, pregnant women, the elderly, and people with disabilities.

Medicare

Medicare is a federal health insurance program for people 65 and older and certain younger people with disabilities. It also covers people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).

CHIP

Children's Health Insurance Program (CHIP) is an insurance program that provides low-cost health coverage to children in families that earn too much money to qualify for Medicaid but not enough to buy private insurance. In some states, CHIP covers pregnant women.

WIC

The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides federal grants to states for supplemental foods, health care referrals, and nutrition education for low-income pregnant, breastfeeding, and non-breastfeeding postpartum women, and to infants and children up to age 5 who are found to be at nutritional risk.

SNAP

Supplemental Nutrition Assistance Program (SNAP) provides food benefits to low-income families to supplement their grocery budget so they can afford the nutritious food essential to health and well-being.

Health Texas Women

The Healthy Texas Women program offers free women's health and family planning services to eligible, low-income women.

Housing and Utility Programs

Assistance for low-income households in meeting their housing and utility needs.
